Erythrocyte sedimentation rate (esr) as a predictor of cardiovascular death. the reykjavik study

Abstract

Background: Inflammation has been increasingly implied in the process of developing cardiovascular disease. ESR is a well known indicator of inflammation, both acute and chronic. Objective: To evaluate ESR as a predictor of risk of cardiovascular death. Methods: In the Reykjavik Study 9, 139 men and 9. 773 women, born 1907-1935, were examined in 5 stages according to a standardized protocol and followed for up to 27 years (mean: 13y). The endpoint was death from coronary heart disease (CHD) or stroke. ESR was measured according to the method of Westergren. The transform ln(ESR+1) was used as a predictor in Cox's regression where age and calendar year at examination were included for adjustment. The hazard ratios shown refer to doubling of ESR+1. The risk of death from CHD or stroke was estimated with the follow-up starting at three different points in time: 1) at the time of ESR measurement, 2) 5 years later. 3) 10 years later. Results: The ESR was found to be a clear risk factor for death from CHD and stroke. With a doubling of the ESR-value the risk of CHD-death increased 30% and of stroke death 20%.
